Specialist Visit Preparation

New patients need to complete our forms and provide essential information prior to consideration of their acceptance by the practice. 

Proof of valid insurance information and authorization need to be provided from PCP's office.

Appointments are given only after the acceptance. 

You must have a  State ID or driver's license with you at the time of the appointment. 

To limit transmission of diseases to our patients and visitors we request you to not have more than one person accompany you at the time of the office visit.  

Established patients need to provide any additional record of care after the last visit.

Please be aware that we do not write any narcotic prescriptions.

Consultations are by appointment only and only after acceptance by the practice. No narcotics are prescribed. 

We are not set up to handle emergencies, so in case of an emergency please contact your PCP/ go to an emergency room.
