New patients are required to complete the new patient forms prior to their acceptance.
So please make sure that you have completed all the forms and agreements and submitted them and have valid insurance information entered correctly for acceptance by the practice.
Appointments are given only after the acceptance.
Please bring your valid health insurance card, updated medication and allergy lists, and your state ID or driver's license with you. You cannot be accompanied by any children. Only one additional person can accompany you into the waiting room.
Established patients need to bring any additional record of care between the two visits.
Please be aware that we do not write any narcotic prescriptions.
Please come 30 minutes before your appointment time to avoid delay in getting seen. Patients who have completed their paperwork before their appointment time and are ready to be seen will be given priority. If you have not completed the paperwork prior to the appointment time, you may be skipped. Late arrivers may need to reschedule their appointment after 20 minutes of being late.

We are located in that building.
Consultations are by appointment only and only after acceptance by the practice. No narcotics are prescribed.
We are not set up to handle emergencies, so in case of an emergency please contact your PCP/ go to an emergency room.
When you need a real specialist doctor, let us know.
Your GI issue, insurance information and pertinent medical records are needed for the practice to determine acceptance.
